I agree to the following terms:
- Toys donated by Toys for Tots will not be auctioned, sold or otherwise turned over for monetary donations.
- Toys donated by Toys for Tots will not be taken out of state for distribution.
I understand any violation of the above agreement will result in disqualification from toys for tots.
We are seeking your help to make this program as successful as possible and fair to all children. Please do not inflate the number of children on your toy request form. We do our very best to make sure each child between the ages of newborn through 13 years receives at least two toys. We ask you to submit a request to only one distribution center.

You are requested to bring a current Photo Id, proof of childs age, such as Birth certificate, a current bill with your address on that matchs your Id.
